The Bosniak classification of cystic renal masses (version 2019), or usually simply Bosniak classification, version 2019 , is a proposed update of the classic/current Bosniak criteria. The update hopes to improve on the classification's predictive power for malignancy and minimize the number of benign lesions undergoing treatment.
Jan 4, 2021 Most kidney cysts are simple kidney cysts, thin-walled sacs filled with fluid. You might have to have treatment if your cyst bursts and bleeds,
Simple renal cysts are commonly observed in normal kidneys, with an increasing incidence as individuals age . They are benign, asymptomatic lesions that rarely require treatment. However, complex cysts may require follow-up imaging, biopsy, or surgical excision for diagnosis. Indeed, Bosniak himself has advocated placing borderline II—III lesions, especially hyperdense cysts, into category III. In general, the number of patients in studies evaluating the value of the Bosniak classification in predicting malignancy in a complex cystic lesion is small, particularly for categories II and III lesions [ 1 , 3 , 5 ]. Kidney cysts can impair kidney function, although many are what are called simple cysts which do not result in health complications. Here’s an overview of what kidney cysts are.
2021) have widespread validation. Of these 22 small cysts, 14 showed growth at follow-up (average growth, 0.68 cm; range, 0.1–1.9 cm), and 11 of those were downgraded to a Bosniak category 2 after growth demonstrated increased internal fluid, with decreased internal soft tissue (eg, initial wall thickening or thickened septations became thinner as the fluid contents in the A smooth, thin (≤2-mm) wall is a feature of a Bosniak I cyst, a smooth and thin (≤2-mm) wall and septa are features of a Bosniak II cyst, a smooth and minimally thickened (3-mm) wall or septa is a feature of a Bosniak IIF mass, and a thickened (≥4-mm) enhancing wall or septa is a feature of a Bosniak III mass. Slightly more than half of Bosniak III renal cysts and most Bosniak IV renal cysts are malignant at surgical pathology [1–3, 10, 11]. The standard of care for both Bosniak III and IV renal cysts is surgical excision, although imaging surveillance is an acceptable management strategy in patients with a short life expectancy or significant comorbidities [ 2 , 3 ].

A Bosniak 3 cyst will be cancerous 50% of the time and unless there are co-morbidities that preclude surgery are usually dealt with in a similar fashion to solid renal/kidney masses. If a kidney lesion is a solid mass, particularly one that picks up blood and thus “enhances” on contrast CT, it is considered malignant until proven otherwise.
My doctor says that I have a Bosniak 3 or 4 kidney cyst, and I should have it removed. How is this surgery performed? Given the 50-90% risk of cancerous involvement in Bosniak 3-4 kidney cysts, it is best to have these removed surgically. This diagram depicts various renal cysts morphologies, listed in order by their potential for malignancy, using the Bosniak classification system.
